Initial Evaluations:

  • Deposit before psychiatric evaluation of $150 (payment can be made over the phone when scheduling session).  We request only 24hr notice to schedule an initial evaluation and will schedule you for an appointment as soon as possible.  We will always remind you of your appointment one to two days before hand, you may cancel at this time at no charge if needed.  The evaluation deposit will be returned or credited if 24hr notice of a cancellation is provided.

Follow-up Appointments:

  • Follow-up appointments will generally be scheduled at the end of each session.  In the event that you are unable to schedule at the time, or need to change your appointment, you may contact us through the main practice number and reschedule with our assistant.  We will always contact you with a reminder of your upcoming appointments.

Rescheduling and Missed Appointments:

  • If you need to reschedule any of your appointments, we request 24hrs notice.  You may contact us through the main practice phone number.
  • We request 24hr notice to cancel any appointment with our clinicians.  We will always contact you (by phone or email, whichever you prefer) one to two business days prior to your appointment for a reminder and so that you may reschedule without charge if necessary.
  • Missed appointments will be billed according to the rates for the service that was scheduled.

Insurance:

  • If you wish to utilize insurance benefits, we will provide you with a “superbill” so that you may submit a request for reimbursement.  The type of reimbursement that you are eligible for will depend on your insurance, network constraints and the nature of your deductible for services.
  • The providers within our practice are out of networks and plans.  The reasons for this are complex; basically, we can practice without restrictions on the length of sessions or the number of visits we can see you.  Dr. Oddy is not restricted by very short visits and we can provide the highest standard of care this way.

Overdue Accounts / Other Fees :

  • Accounts that are past due may be referred to a collection agency as a last resort.  Any fees, losses or charges including collection fees or legal fees, up to 50% of the amount owed, made while securing past due balances will be added to any overdue accounts.
  • Checks that are returned as unpaid by your bank will result in a $30.00 charge.
  • In regards to requests for responding to a subpoena and requests for records or summary of your records, there is a fee for each that will be added to your account.
  • If you are having financial difficulties that make payment difficult, please let us know.  Sometimes, financial issues are one of the hardest things for a patient to discuss.  The more this is an open issue, the more we can think together to agree on a plan.
